COVID-19 - Registrar of Contractors Remains Open for Business - In-Person Services by Appointment Only Read More

The CDC recommends continued physical distancing and in Phase 1 and 2, the ROC Offices will remain open to the public by appointment only. During this time, you should know you can interact with the ROC online and as an Agency, our staff continues to process license applications and changes to licenses and complaints also continue to move through the administrative process. For contact information and additional information related to any process changes at ROC at this time, please visit our Services Related to COVID-19 tab. Though we encourage you to use online services or give us a call at (877) 692-9762 at this time, the Phoenix and Tucson Offices are open to the public by appointment only.

If you're filing an application for a new license or a complaint to be investigated, please know you can use our online portal. The Online Portal can be accessed from the ONLINE SERVICES tab.

If you need to renew your license, please know you can do so online at HERE.

OTHER CHANGES TO YOUR LICENSE?

If you need to change information related to your license (the qualifying party, address, contact info, bonding information, etc), you are welcome to complete the appropriate form - found on the FORMS page under the Contractor Resources tab - and email it to [email protected]. If there are any fees related to the change, include a contact phone number and the Licensing Department will reach out to you to walk you through the payment process.

Any change to information provided in order to obtain a contractors license must be provided to the Registrar of Contractors within 30 calendar days of the change.

    Use this form to change the company mailing address, this must be done within 30 calendar days of the move. Mail original form with original signature to P.O. Box 6688, Phoenix, AZ 85005-6688.

  • Assumption RC-L-242
    Use this form to assume responsibility of any license being canceled when applying for a new license. One (1) copy is required for each license being applied for, to be sent to any office of the Arizona Registrar of Contractors.

    You may furnish this form to your insurance company, licensed to write bonds in the State of Arizona. See bond instructions below.

    You may wish to read these instructions before acquiring a license bond.
  • License Cancellation Form RC-L-201A
    Use this form to cancel (terminate) a license. One (1) copy may be sent to any office of the Arizona Registrar of Contractors, that single copy may list all of the licenses you wish to cancel. A canceled license can not be reactivated.

  • Exemption from Qualifying Party Requirements Application RC-L-280
    Use this form to apply for a qualifying party exemption. Each license requires a separate form and fees. Be sure you meet all of these requirements before applying for an exemption.
  • License Inactivation Form RC-L-201B
    Use this form to place a license inactive. One (1) completed form is required for each license to be placed in inactive status. Inactivating a license keeps it from expiring during a period when it is not intended to be used. It can then be reinstated within five (5) years of the inactivation date.
  • License Classification Change Form RC-L-201C
    Use this form to opt-out of a dual license classification issued to your business.
  • Name Change Request RC-L-300
    Use this form if you wish to change the company name listed on your license, one (1) copy is required to be sent to any office of the Arizona Registrar of Contractors.
  • Name Change Request for Certificates of Deposit RC-L-293
    If a Certificate of Deposit is held in lieu of the surety bond, include this form instead of a bond rider. Please include one completed form for each license number.
  • RC-L-201F Add Ownership/Personnel Form
    Use this Add Ownership/Personnel Change Form to add owners and personnel on an existing license.

  • Tiered Entity Organization Chart RC-L-205D
    This form is for use by a tiered-entity applicant to comply with A.R.S. § 32-1122(B)’s application disclosure requirements. A Tiered Entity is an entity that is owned or operated by another entity.
    • For example, if applicant, “Red Corporation,” is owned or operated by “Blue, LLC,” “Red Corporation” would be considered a Tiered Entity for the purposes of obtaining a contractor’s license.
  • RC-L-201G Ownership/Personnel Update/Remove Form
    Use this Update/Remove Ownership/Personnel Change Form to remove owners and personnel on an existing license. You may also use this form to update personnel and contact information on an existing license.

  • Application for New Qualifying Party RC-L-201D
    Use this form for replacing the qualified party of any license. One (1) copy is required for each license the qualifying party is being changed on, to be sent to any office of the Arizona Registrar of Contractors.
  • Notice of Disassociation/Resignation RC-L-305
    This form must be received by the Registrar within 15 days of the date the Qualifying Party is no longer connected to the license in that capacity. The licensee must re-qualify through another person within 60 days of the date of disassociation or the license will be automatically suspended until a new Qualifying Party is approved by the Registrar.

  • Application to Reactivate License RC-L-222
    Use this form to reinstate a license that has been placed on inactive status. One (1) copy is required for each license to be reinstated, and the Arizona Registrar of Contractors must have 30 days notice before the license is due to expire.
  • Request for Affidavit RC-L-303
    Use this form to request an affidavit with regards to information concerning any license. One (1) copy is required, to be sent to any office of the Arizona Registrar of Contractors with a $10.00 processing fee.
  • Scope Determination Request Form RC-L-203
    After researching our existing license classifications here, if you're still not sure what license you need, you may use this form to request a license classification determination from the Registrar.

  • Petition for Review of Criminal Record Form RC-L-100
    Use of this form is not required for application but enables the agency to make a determination whether the applicant's criminal history would preclude licensure prior to submitting an application.
